Information For Youth Sports Coaches & Instructors

Coach Requirements

As a coach, there are a few required forms and documents that you will need to complete. If team registration is handled by Village Life, coaches only need to submit the top 3 items (in bold).

Coach Requirement List

Background Check

All head and assistant coaches are required to have a current background check on file. This is good for 3 years. Contact Village Life if you have questions about the status of your background check.

When you have committed to coach and you need a background check, you will receive an email from Corporate Intelligence Consultants. In the email will be a link where you can submit your information online.

If your background check is not paid for by the program you are coaching for, you will need to submit payment ($32). The fee may be paid by check payable to 'Village of Ottawa Hills' or paid online Here.

Concussion Training

All head and assistant coaches are required to go through the free online concussion training course. Once completed, you will receive a certificate that is good for 3 years. If you have not taken the course or your certificate is older than 3 years, please click HERE. Once completed, submit your certificate to Village Life.

In addition, according to the Ohio Department of Health, a concussion information sheet must be distributed to all parents of children participating in youth sports. If registration is done through Village Life, this will be included with registration. All coaches who handle their own registration must distribute this required information to their athletes and their parents. The concussion information sheet may be found below.

Lindsay's Law (Sudden Cardiac Arrest)

All head and assistant coaches are required to watch a video and review the informational handout. When all requirements have been met, you will need to submit the signed checklist to Village Life.

Lindsay’s Law, Ohio Revised Code 3313.5310, 3707.58 and 3707.59 went into effect in 2017. In accordance with this law, the Ohio Department of Health has developed guidelines and other relevant materials to inform and educate youth athletes, their parents, and their coaches about the nature and warning signs of sudden cardiac arrest. All coaches who handle their own registration must distribute/collect the required information to/from their athletes and their parents. If registration is handled through Village Life, this information is already included.

Team Roster

A complete roster must be submitted to Village Life at the beginning of your season.

Insurance

Along with your roster, the insurance money must be paid to Village Life ($5 per player) at the beginning of your season. If registration is done through Village Life, these fees are included in the registration cost. All coaches who handle their own registration must submit payment of these fees to Village Life. These fees may be included in your team fees so you may recoup the expense. Checks may be made payable to the 'Village of Ottawa Hills' or contact Village Life to have an online payment option set up for you.

Facility Request Form

A facility request form must be submitted to reserve space (inside or outside) for practices or games. Forms may be accessed in the link below. There are 2 versions - pdf and word doc. The word doc is a fillable form, so you may enter your information and email it to Village Life.

Instructors

As with coaches, there are certain requirements that each Instructor must complete prior to teaching a class or program through Village Life. The list of requirements includes:

  • Background Check
  • Concussion Training*
  • Lindsay's Law Review (Sudden Cardiac Arrest)*
  • Signed contract with Village Life
  • Independent Contractor Form
  • W-9

Instructor Requirement List

How to Form a Team

There are several different ways of forming teams for Ottawa Hills youth sports - from tryouts to signups. To start a team, there must be communication to the appropriate age/gender group. This starts with publication of a tryout date or collecting names through an interest form. Once it has been properly communicated, teams may have their registration go through the Village Life class page or go directly through the coach.

To start a team:
- Contact Village Life and give details of sport, age group, season dates, etc.
- Coaches must complete all the coaching requirements, listed above.

In order to schedule field or gym time, a team must be formed following the above guidelines. In addition, in-season sports receive priority for field/gym time over out of season sports. The high school varsity sports schedule is used to determine each sport season.
