INFORMATION FOR ALL YOUTH SPORTS COACHES

As a coach, there are a few required forms and documents that you will need to submit to Village Life or to your team. Below is a checklist.

COACH REQUIREMENTS 

BACKGROUND CHECK - All head and assistant coaches are required to have a current background check on file.  This is good for 3 years.  Contact Village Life if you have questions about the status of your background check. 

There are 2 ways to apply for a background check: 

  • Submit this form to the Office of Village Life or
  • Email your legal name to villagelife@ohschools.org and you will receive a link to submit your information online

The cost is $32. The fee may be paid by check payable to Village of Ottawa Hills or paid online HERE

CONCUSSION TRAINING - All head and assistant coaches are required to go through the free online concussion training course.  Once completed, you will receive a certificate that is good for 3 years.  If you have not taken the course or your certificate is older than 3 years, please click HERE.   Once completed, submit your certificate to Village Life.  

{Hint:  Once you click on the link above, enter your state where requested and "Order Course".  Sign in to your account (or create an account) and once logged in, you may either access the course in your cart or find it in "My Courses" under the "Dashboard" tab.}

In addition, according to the Ohio Department of Health, a concussion information sheet must be distributed to all parents of children participating in youth sports.  Please include this information with any registration or information that you distribute to your team.  The concussion information sheet may be found in the left sidebar.

LINDSAY'S LAW (Sudden Cardiac Arrest) - All head and assistant coaches are required to watch a video and review the informational handout. When all requirements have been met, you will need to submit the signed checklist to Village Life.

In accordance with this law, the Ohio Department of Health has developed guidelines and other relevant materials to inform and educate students and youth athletes participating in or desiring to participate in an athletic activity, their parents, and their coaches about the nature and warning signs of sudden cardiac arrest. The following resources were developed to implement Lindsay’s Law:

All coaches who handle their own registration must distribute/collect the required information to/from their athletes and their parents. 

TEAM ROSTER -  A complete roster must be submitted to Village Life at the beginning of your season.

INSURANCE - Along with your roster, the insurance money must be paid to Village Life ($5 per player) at the beginning of your season.  Checks may be made payable to the Village of Ottawa Hills.  These fees may be included in your team fees. 

FACILITY REQUEST FORM - A facility request form must be submitted to reserve space (inside or outside) for practices or games.  Forms may be accesssed in the left sidebar of this page.  There are 2 versions - pdf and word doc.  The word doc is a fillable form, so you may enter your information and email it to Village Life.
